Acadia Center for Nursing and Rehabilitation, formerly known as Riverhead Care Center, Acadia Center for Nursing & Rehabilitation has proudly served the East End of Long Island for more than 40 years. Conveniently located in Riverhead, New York, we provide skilled nursing, rehabilitation, and long-term care services to residents throughout the North Fork, including Calverton, Baiting Hollow, Jamesport, Laurel, Mattituck, Cutchogue, Peconic, Southold, Greenport, East Marion, and Orient Point.
We also proudly serve surrounding communities, including Hampton Bays, Westhampton, Southampton, East Hampton, Quogue, East Quogue, Moriches, East Moriches, Center Moriches, Manorville, Shoreham-Wading River, Rocky Point, Ridge, Coram, Middle Island, Yaphank, Patchogue, Medford, Brookhaven, Bellport, Shirley, Mastic, and Mastic Beach.
To ensure a seamless continuum of care, we work closely with Northwell Peconic Bay Medical Center, located just a half-mile from our facility, as well as numerous home care agencies throughout the region. Our collaborative approach helps make transitions between hospital, rehabilitation, home care, and long-term care as smooth and efficient as possible.
